Conditions and Treatments

Conditions

Our clinical practice specializes in the diagnosis and treatment of skin, hair and nail disease. Some of the conditions we treat include skin cancer, acne, skin blemishes, moles, warts, hair and scalp disorders, facial blood vessels, birthmarks, eczema, dry skin, skin allergies, nail problems, wrinkles, frown lines, and childhood skin conditions. We continually search for the latest technology and most beneficial treatments. Our methods of treatment include surgical dermatology (surgical excision), electrosurgery, cryosurgery, laser surgery, phototherapy (NBUVB and PUVA), wrinkle fillers and BOTOX.

Dermatology: the treatment of skin, hair and nails. The largest organ of the body.

A Brief Description of Conditions we treat:

  • Acne

Acne is a disease that affects not only teens but adults as well. It generally appears on the face (nose, cheeks, chin), chest and back. You may call them pimples, zits, blackheads, whiteheads and, in more severe forms, large and sometimes painful cysts. Acne occurs when the pores of the skin become clogged with oil, dead skin cells and bacteria. At first, very mild cases can be treated with over-the-counter acne medications containing benzoyl peroxide, but if this does not resolve the problem, you should consider an appointment with the physicians at Associated Dermatologists in West Bloomfield, Commerce or Novi. Multiple medications, both topical and oral, are effective in controlling this condition that if left untreated can result in permanent scarring. In the case of severe, scarring nodular and cystic acne, the medication istotreinoin (better known as Accutane) is a treatment that if often considered.
  • Rashes

All rashes are not equal: Atopic Dermatitis, Eczema, Contact Dermatitis, Poison Ivy, Hives, Fungus, Psoriasis--A trained dermatologist will be able to identify what kind of a “rash” you have and how it should be treated. Our physicians can perform biopsies, take cultures or immediately identify a rash by the clinical presentation. If you have a rash that does not clear or is extremely itchy or spreading, you can make an appointment with a physician or physician assistant at any of our three locations to get you on the path to healthy skin.
  • Warts

Warts are a common skin virus that can appear in many areas of the body, but primarily they appear on the hands and feet. Viruses require your body’s immune system to help fight the infection, and often times several treatments are necessary for clearing. Warts can be contagious and therefore spread. If over-the-counter wart treatments are unsuccessful after a few weeks, it is best to seek the expertise of a physician who treats warts with a multitude of treatment options: liquid nitrogen, topical medications, laser and sometimes injections.

HOW OFTEN SHOULD I HAVE A FULL BODY SKIN EXAM?

Skin cancer is always easier to treat when it is detected early. We recommend full body skin exams yearly to all patients to search for early skin cancers. For patients with a history of skin cancer, we recommend more frequent skin exams.

  • Skin Cancer (the American Academy of Dermatology)

When was your last “skin exam”? Although skin cancer can develop on people of all ages and skin types there are particular factors that increase your risk:
    • Fair skin
    • Skin that burns or freckles
    • Blond or red hair
    • Blue or Green eyes
    • More than 50 moles
    • Irregularly-shaped or darker moles
    • Use (or used) indoor tanning devices
    • History of sunburns, (especially blistering)
    • Personal or Family history of skin cancer
    • Weakened immune system
    • Area of the skin that has been badly burned
    • Received x-ray treatments for acne or long term x-ray therapy
The physicians and physician assistants can do a thorough examination of your skin and if necessary biopsy any suspicious growths.

Basal Cell Carcinoma

This is the most common type of skin cancer and can appear on the skin in many shapes and sizes. If you have a lesion that continues to bleed, a “pimple” that won’t resolve, a sore that heals and then returns you should have your Dermatologist evaluate it. Basal Cell Carcinoma (BCC) usually develops in sun exposed areas- such as the nose, scalp neck ears and hands. Bcc rarely spread to other areas of the body but can become larger and affect the surrounding tissue and sometimes into the bone.

Treatment for a non-invasive BCC can be as simple as a procedure in the office with electrodessication and curettage. But a nodular or invasive BCC may require more extensive, yet more definitive treatment such as Mohs surgery. (See the section regarding this procedure)

Squamous Cell Carcinoma

Squamous Cell (SCC) is the second most common type of skin cancer and can appear as a crusted or rough bump that grows and bleeds and like a BCC can appear to be a sore that doesn’t heal. It is also commonly found on the areas of sun exposure: face ears, lips, back of the hands and arms. But in addition, SCC can appear inside the mouth or on the genitals and smoking or chewing tobacco can increase your risk of SCC in the mouth or throat. If left untreated, SCC’s can spread and therefore make future treatment difficult. Mohs surgery is a common treatment for this skin cancer.

Melanoma

This is the deadliest form of skin cancer. It may develop on normal skin or in an existing mole.

The ABCD and Es of Melanoma

 A: Asymmetry
 B: Irregular Border
C: Varied Colors or very dark
D: Diameter- greater than an eraser on a pencil (6mm) but can be smaller
E: Evolving- a mole or skin growth that looks different from the rest or is changing in size shape or color.

Make an appointment with a Dermatologist as soon as possible if you notice any of these new or changing lesions.

Dermatologists treat many different diseases of the hair skin and nails. In addition to clinical treatments, such as biopsies, surgeries, cryotherapy, and prescription medications Dermatologists also offer many cosmetic solutions to sun damage and aging skin, including laser treatments, fillers, botox or dysport, acid peels, laser hair removal, medical-grade cosmetic products and most recently a procedure known as microneedling.

For information about cosmetic solutions, please make an appointment with any of our estheticians – who will discuss your concerns, your expectations, outcomes and pricing at all three locations for your convenience

Treatments

Accutane

Isotretinoin (Accutane) is a medicine that revolutionized the treatment of acne. Accutane belongs to the family of medicines called retinoids, which are similar to vitamin A. Accutane, like other retinoids, works by altering DNA transcription. This affect decreases the size and output of sebaceous glands. It also makes the cells that are sloughed off into the sebaceous glands less sticky, and therefore less able to form blackheads and whiteheads (comedones). It also reduces the number of bacteria in the sebaceous gland and on the skin surface.

Who Takes Accutane?
Accutane is generally used for nodular, pustular acne that has not responded to full courses of several oral antibiotics. The trend in Accutane prescribing for acne has been towards using it earlier in the course of the disease, especially if there is significant scarring.

Facial Capillary Therapy

Damaged facial capillaries are small red or bluish lines visible just under the skin which can create a reddish, blotchy look. This may also result in vascular flushing, such as with rosacea. We have several treatment options that eliminate redness and produce exceptional results for a wide range of patients including our vascular laser and intense pulse light (IPL) therapy.

This treatment is for:

  • Photo Damage
  • Rosacea

This treatment uses:

Laser Photorejuvenation

Photo damage can manifest as mottled pigmentation, fine wrinkling and thinning of the skin, and vascular dilation and flushing.  Laser and intense pulse light therapy (IPL) can eliminate years off of the skin of the face and chest by improving skin texture, color and laxity. These specialized non-ablative techniques gently treat small blood vessels in the dermis beneath the skin quickly and painlessly. They can enhance skin texture and reduce redness while decreasing uneven pigmentation. They have also been observed to stimulate collagen production, thereby minimizing wrinkles and reducing the appearance of pore size.

This treatment is for:

  • Photo Damage
  • Rosacea
  • Wrinkles

Perlane® Restylane® Juvederm®

The body naturally produces hyaluronic acid that adds volume and smoothness to skin. Perlane® is a hyaluronic acid gel that can be used to treat severe facial wrinkles, such as nasolabial folds (lines from the nose to the corners of the mouth), filling and smoothing the skin. Treatments typically last for about six months and can also be applied to the lips, cheeks, jowls, and other sagging facial skin.

This treatment is for:

  • Photo Damage
  • Wrinkles

Radiesse

Radiesse® dermal filler may be used in an array of treatments ideal for correcting the signs of aging or adding natural contours to the face – without invasive surgery.

RADIESSE dermal filler is setting a new standard for cosmetic fillers. It uses advanced, calcium-based microsphere technology to deliver soft, natural-looking results you can see immediately. And unlike other dermal fillers, it also stimulates your body to produce new collagen, so you’ll continue to look more youthful and attractive over time. In fact for many patients, the benefits of treatment with RADIESSE dermal filler may last up a year or more.

This treatment is for:

  • Photo Damage

Biologics

Only a qualified health care professional can determine whether or not you have plaque psoriasis. Although your family doctor may be able to diagnose your symptoms, it's a good idea to see a dermatologist if you suspect you have psoriasis. Dermatologists specialize in treating skin diseases and can work with, or as a consultant to, your physician. Because dermatologists treat a range of skin conditions, it may be helpful to find out if yours treats psoriasis before you make an appointment.

Humira™
Hate psoriasis? Love clearer skin. You know the challenges can be more than skin deep. Plaque psoriasis can mean red, scaly and sometimes painful skin, which can have a real impact on you. But now there is another option for some people suffering from psoriasis. HUMIRA is used to treat moderate to severe chronic (lasting a long time) plaque psoriasis in adults who are under the ongoing care of a physician, have the condition in many areas of their body, and who may benefit from taking injections or pills (systemic therapy) or phototherapy (treatment using ultraviolet light alone or with pills). When considering HUMIRA, your physician will determine if other systemic therapies are medically less appropriate. In one clinical trial for plaque psoriasis patients, 7 out of 10 adults taking HUMIRA saw 75% skin clearance and 6 out of 10 patients had clear or almost clear skin at 16 weeks. Individual results may vary. HUMIRA is available by prescription only. Talk to your dermatologist today about your treatment options and if HUMIRA may be right for you.

Enbrel™
In patients with plaque psoriasis, there is an overproduction of skin cells. Normally, skin cells are formed underneath the surface of the skin and move up to the surface over a period of about 30 days. These cells are eventually shed, revealing new skin cells. In people with plaque psoriasis, however, this skin cell cycle is sped up so that the skin cells multiply in just 3 to 4 days. The body has no way to shed the skin cells fast enough, and they accumulate on the surface, forming raised, red patches or lesions. ENBREL works in a targeted way inside your body, on the immune system, to help slow the rapid growth of skin cells and reduce the inflammation associated with moderate severe plaque psoriasis. In other words, ENBREL works from the inside out. Because ENBREL works on the immune system, it may lower your ability to fight infection.

 Xtrac or Pharos Laser

Using a carefully focused beam of laser light delivered through a sophisticated liquid light guide delivery system, the XTRAC system is designed to clear unsightly psoriatic skin plaques quickly and effectively. Because it concentrates light on active lesions, XTRAC allows your health care technician to deliver the high therapeutic doses necessary for rapid clearing without risk to healthy skin.

Sclerotherapy

To treat varicose and spider veins, a small needle is inserted, applying a medication that collapses the vein upon itself. The body then absorbs the shrunken tissue, removing the diseased vein.

This treatment is for:

  • Spider Veins

Laser Hair Removal

Facial and body hair removal is faster and easier than ever thanks to new laser technology. With a few short sessions, we can eliminate approximately 80% of unwanted hair to create a smoother, low maintenance look. For women, the most common areas for hair removal are the upper lip, chin, neck, cheeks, bikini line, legs, underarms and around the navel. For men, we typically remove hair on the back, neck, shoulders and ears.

This treatment is for:

  • Unwanted Hair

BOTOX® Cosmetic

BOTOX® Cosmetic is the trade name of Botulinum Type A, a powerful muscle toxin protein produced by Clostridium botulinum bacterium. When injected in tiny amounts directly into a specific muscle, an effect called botulism occurs, only relaxing the action of that muscle. This procedure is very effective at softening the lines that come from facial movements like frowning and squinting, tightening the facial skin.

Our BOTOX® experienced and trained physicians administer treatments and know exactly how to banish those wrinkles from your face. BOTOX is a quick, effective way to get rid of wrinkles.

ActiveFX™ -- Lumenis UltraPulse Encore CO2

ActiveFX yields remarkable, long lasting results with just one treatment. A fast, office-based procedure with minimal patient downtime, ActiveFX is your single-treatment solution for wrinkled, sun-damaged skin.

A high-energy laser beam is applied in a fractional pattern over the surface of the treatment area, leaving areas of untouched skin to promote quicker healing.

Is ActiveFX right for you?
ActiveFX is an ideal treatment option if you:

  • have a busy lifestyle or cannot be away from work for a long time
  • want noticeable results with minimal downtime
  • want to treat dyschromia, fine lines and wrinkles

What can you expect following ActiveFX?
You may feel warmth similar to sunburn following your ActiveFX treatment. You can drive yourself home after the procedure. With a gentle washing and moisturizing regimen, your phototreated skin will begin to flake off within 2 days. After about 4-7 days, your doctor will probably let you wear make-up. Pinkness is often gone in about a week.

What is ActiveFX?
With ActiveFX treatment, a high-energy beam of laser light interacts with the skin’s surface causing the upper layer to peel off and regenerate a layer of new, healthy skin. The laser beam is applied in a fractional pattern, leaving small “bridges” of untouched skin. This technique makes the healing process much faster and enables you to get back to normal activities sooner.

ActiveFX Benefits

ActiveFX produces a dramatic effect on the skin:

  • Causes immediate collagen contraction
  • Reduces wrinkles and fine lines, and softens deeper frown lines
  • Noticeably improves skin tone and texture
  • Stimulates new collagen formation and plumps the skin

Results of treatment can be maximized with proper sun protection. Most effects of treatment become visible right away, whereas others—such as new collagen formation—build up gradually and results in continued smoothing over time. Therefore, most people continue to show improvement up to six months after the procedure.

This device is used to treat:

  • Photo Damage
  • Wrinkles

DeepFX™ -- Lumenis UltraPulse Encore CO2™

What is DeepFX?
DeepFX is a revolutionary fractional laser technology for aged, sun-damaged or scarred skin. The low-downtime procedure uses fractional CO2 laser microbeams to precisely target the deep dermal layers of the skin. This stimulates a cascade of healing responses that leads to tissue regeneration and new collagen formation throughout the entire treatment area. And because only a fraction of the skin is actually touched by the laser, healing time can be limited to just a few days.

How does DeepFX work?
DeepFX procedures involve treating only a fraction of the skin’s surface, leaving bridges of untouched skin to aid in the regeneration process. Each laser pulse removes a narrow column of tissue while simultaneously treating surrounding skin. This deep heating causes immediate collagen contraction and long-term collagen remodeling. The result is softening and smoothing of wrinkles and scars, and improved skin texture.

A high-energy laser beam is applied in a fractional pattern over the surface of the treatment area, leaving areas of untouched skin to promote quicker healing.

DeepFX is typically performed in a physician’s office and lasts for about 20 minutes. Your doctor may use an anesthetic cream prior to treatment to aid with comfort.

Where can DeepFX be used?
DeepFX is commonly used on the face, around the lips, for crow’s feet, the neck, upper chest, hands and arms. Ask your doctor if DeepFX is right for you.

Who can get a DeepFX treatment?
DeepFX is safe for most people; however it shouldn't be used if you have taken Accutane™ in the past 12 months, have a history of keloid formation, have a history of poor wound healing, or demonstrate excessive or unusually prolonged erythema, hyperpigmentation or hypopigmentation upon laser test patching.

DeepFX Results

DeepFX produces a dramatic effect on the skin:

  • Noticeably improves skin tone and texture
  • Stimulates new collagen formation and plumps the skin
